GLP-1 Side Effects: How to Manage Nausea, Vomiting, and Other Common Reactions

Dr. Arpana Pillay

Internal Medicine Physician, Vish Medical

GLP-1 receptor agonists — including semaglutide (Ozempic, Wegovy) and tirzepatide (Mounjaro, Zepbound) — have transformed medical weight loss, but they come with a notable side effect profile that causes a significant minority of patients to discontinue treatment. Understanding these effects, why they occur, and evidence-based strategies to manage them is crucial for staying on track with your physician-supervised weight loss program.

The Most Common GLP-1 Side Effects

GLP-1 medications slow gastric emptying and act on areas of the brain that regulate nausea and appetite. This mechanism explains both their weight loss efficacy and their gastrointestinal side effect profile. The most common adverse effects include:

  • Nausea: Affects 20–40% of patients, most prominent during titration; typically improves over weeks
  • Vomiting: Less common than nausea; usually occurs with overeating or consuming high-fat meals
  • Constipation: GI motility slows with GLP-1 use; affects roughly 15–25% of patients
  • Diarrhea: Some patients experience loose stools, particularly early in treatment
  • Belching or bloating: Related to delayed gastric emptying
  • Fatigue: Common in early weeks, usually resolves with dose stabilization
  • Injection site reactions: Mild redness or swelling at injection site — rotate sites to minimize

Practical Management Strategies

  • Eat small, frequent meals rather than large portions — GLP-1 significantly slows gastric emptying
  • Avoid high-fat, fried, or very spicy foods, especially early in treatment
  • Stay well-hydrated — adequate fluid intake reduces nausea and prevents constipation
  • Take semaglutide at bedtime if nausea is problematic during the day
  • Slow titration: do not rush dose increases if current dose is not tolerated
  • For constipation: increase fiber intake, stay active, and discuss stool softeners with your physician if needed
