Primary Care vs. Urgent Care vs. ER: Which One Do You Actually Need?

Dr. Arpana Pillay

Internal Medicine Physician, Vish Medical

One of the most common healthcare decision points is choosing between primary care, urgent care, and the emergency room. Making the wrong choice leads to unnecessary expense, inappropriate care, and long waits. This guide helps you navigate the decision for the most common health situations so you get the right care at the right time — and at the right cost.

When to See Your Primary Care Physician

Your primary care physician should be your first call for the vast majority of health concerns. Primary care is the right setting for annual physicals and preventive screenings, management of chronic conditions (diabetes, hypertension, thyroid disease), medication management and refills, routine lab work and follow-up, mental health screening and referrals, and new or ongoing symptoms that are not urgent. Establishing care with a PCP before you need urgent care also means someone who knows your history is involved in your care decisions.

At Vish Medical, Dr. Arpana Pillay offers evening and Saturday appointments plus telehealth Monday through Friday evenings, making primary care accessible without sacrificing your workday.

When Urgent Care Is Appropriate

  • Minor injuries (sprains, strains, minor lacerations needing stitches)
  • Illness symptoms when your PCP is unavailable (same-day)
  • Mild to moderate infections that need evaluation same-day
  • Minor burns, rashes, or skin concerns needing immediate treatment
  • Testing needs (rapid strep, flu test) when your PCP cannot see you

When the Emergency Room Is the Right Choice

True emergencies require emergency care: chest pain or pressure, severe shortness of breath, signs of stroke (facial drooping, arm weakness, speech difficulty), severe abdominal pain, head injury with loss of consciousness, high fever with stiff neck, severe allergic reactions (anaphylaxis), and active uncontrolled bleeding. The ER is expensive and crowded — but for these situations, it is exactly the right choice. When in doubt, call 911.

For most non-emergency needs, Dr. Pillay at Vish Medical provides same-day and next-day appointments, including telehealth. Book your appointment today.
