Wegovy vs. Ozempic: What's the Difference and Which One Is Right for Weight Loss?

Ozempic: The Diabetes Drug That Became Famous for Weight Loss

Ozempic was FDA-approved in December 2017 for the treatment of type 2 diabetes and cardiovascular risk reduction. Available doses are 0.5 mg, 1 mg, and 2 mg weekly. The observation that Ozempic produced significant weight loss in diabetic patients led to the development and approval of higher-dose semaglutide specifically for weight management. Many physicians prescribe Ozempic off-label for weight loss in non-diabetic patients, particularly when Wegovy is unavailable or not covered by insurance.

Wegovy: The Weight Loss Formulation

Wegovy was FDA-approved in June 2021 specifically for chronic weight management, at doses escalating to a maintenance dose of 2.4 mg weekly — higher than the maximum Ozempic dose. This higher dose produces greater appetite suppression and weight loss than lower doses. In the STEP 1 trial, patients on Wegovy 2.4 mg lost an average of 14.9% of body weight at 68 weeks, compared to 2.4% for placebo. Wegovy is indicated for adults with BMI ≥30, or ≥27 with a weight-related condition.

Insurance and Practical Considerations

  • Ozempic: more likely to be covered for patients with type 2 diabetes; less coverage for obesity alone
  • Wegovy: specifically covered for obesity management in some commercial plans; out-of-pocket cost can be $1,300+ per month without coverage
