Essential Women's Health Screenings: A Complete Guide by Age

Dr. Arpana Pillay

Internal Medicine Physician, Vish Medical

Preventive care is one of the most powerful tools women have for maintaining long-term health. Many serious conditions — including breast cancer, cervical cancer, osteoporosis, and cardiovascular disease — are far more treatable when caught early through routine screening. Yet studies consistently show that a significant portion of women are behind on recommended preventive tests. This guide provides a clear, age-based roadmap for essential women's health screenings.

In Your 20s

The 20s are an important decade to establish primary care and begin baseline screenings. Pap smears (cervical cancer screening with Pap test ± HPV co-test) begin at age 21 and are repeated every three years if normal. Blood pressure should be checked at least every two years. STI screening is recommended for sexually active women under 25. Establishing a relationship with a primary care physician who can coordinate your care across specialties is the most important step in this decade.

In Your 30s and 40s

  • Continue Pap smear every 3 years, or every 5 years with HPV co-test starting at age 30
  • Breast cancer awareness — discuss mammogram initiation with your physician (some guidelines recommend starting at 40, others at 50)
  • Blood glucose screening if you are overweight, have PCOS, or have a family history of diabetes
  • Cholesterol and blood pressure monitoring, especially if you have cardiovascular risk factors
  • Thyroid function testing if you experience fatigue, weight changes, or menstrual irregularities

In Your 50s and Beyond

Post-menopausal years bring new screening priorities. Annual mammograms are widely recommended starting at 50 (or earlier based on individual risk). Bone density (DEXA scan) is recommended at age 65, or earlier if you have risk factors like early menopause, prolonged steroid use, or low body weight. Colorectal cancer screening begins at 45 for average-risk individuals. Cardiovascular screening becomes increasingly important as estrogen's protective effects wane after menopause.
